CANCELLATION POLICY

We require a minimum of 4 hours notice to cancel or reschedule an appointment. Short notice cancellation and No Shows will incur a fee of $50 for a regular visit and $150 for longer appointments such as Physicals, Drivers Medicals, Minor procedures, Form Completion.

If you need to cancel and appointment outside of business hours you can 1) call the clinic and leave a cancellation voicemail 2) send a text message to 780-987-3314. You must include your name, date and time of appointment and which doctor you are booked with.

UNINSURED SERVICES

There are many services that are not covered by Alberta Health but involve much administrative work by the physician and / or staff members. Patients are responsible for the fees incurred by these services.


There will be a fee for all notes or forms, this is not an inclusive list. Please ask about the fee before the task is started.


Blue Cross Special Authorization Forms $40

Drivers Medical (Age 74.5 and under) $150

Insurance Forms (based on how extensive the request is) $50 - $200

Transfer of Medical Records (plus $.25/ page over 50 pages) $30 and up

Travel Immunization (per injection) $15


Cosmetic Mole / Skin Tag Removal $100 - $150


Uninsured Cryotherapy (liquid Nitrogen) per treatment $25

HARASSMENT - ZERO TOLERANCE

We understand that everyone we interact with is facing hardships whether they be physical, mental or emotional. Our staff and doctors are no exception.

The staff endeavour to be empathetic and assist patients to the best of their ability. There are limitations to this because they are responsible to follow the direction they receive from the doctors as to how and where to book patients and what type of appointment require in person versus phone appointments. There are also clinic policies that they are responsible to enforce. We understand this can be frustrating as it may not match up with your expectation.

Harassment, threats, verbal or physical abuse will not be tolerated. Patients who exhibit any of this behaviour will no longer be welcome at Devon Medical Clinic.
